Privacy Policy

1. Our Commitment to Privacy

Grimsby Rehab Co is committed to protecting the privacy of your personal health information (PHI). As a health information custodian under Ontario's Personal Health Information Protection Act (PHIPA), we are legally responsible for safeguarding your information. This policy explains how we collect, use, disclose, and secure your PHI. All staff, practitioners, and volunteers are required to follow this policy.


2. Personal Health Information (PHI) We Collect

We collect PHI directly from you or from people acting on your behalf, and sometimes from other health information custodians with your consent or as permitted by law. This information is necessary to provide you with quality care.

PHI we may collect includes:

  • Identification and Contact Information: Name, date of birth, address, phone number, email address, and Ontario Health Card Number.
  • Health History: Medical, family, and social history, records of your visits, symptoms, test results, diagnoses, and treatment plans from all practitioners at our clinic.
  • Billing Information: Insurance information (e.g., WSIB, extended health benefits) and payment history.

3. Purposes for Collecting, Using, and Disclosing PHI

We collect, use, and disclose your PHI for the following purposes:


Provision of Care

To provide diagnosis, treatment, and ongoing care from all practitioners within the multidisciplinary team. We assume your implied consent to share your PHI with your circle of care (e.g., other practitioners and staff involved in your care within our clinic).

Administration & Operations

To operate our clinic, including quality improvement activities, billing and accounting, conducting internal audits, and general administrative functions.

Research & Teaching

With your express consent, your de-identified information may be used for teaching, research, or statistical purposes, or as otherwise permitted by law.

Legal & Regulatory

To meet legal obligations, such as reporting to public health authorities (e.g., reportable diseases) or to regulatory colleges, or in response to a court order or subpoena.

Communicating with You

To schedule appointments, send appointment reminders, and follow up on your care.


4. Your Rights Regarding Your PHI

Under PHIPA, you have the right to:

  • Consent: You have the right to consent to the collection, use, and disclosure of your PHI, and you can withdraw your consent at any time, subject to legal exceptions.
  • "Lockbox" Right (Restriction): You may request that we not use or disclose specific parts of your PHI to other health care providers for healthcare purposes. We will comply with this request unless PHIPA permits or requires otherwise (e.g., in an emergency).
  • Access and Correction: You have the right to examine your health record and receive a copy (a fee may apply). You also have the right to request a correction to your health record if you believe it is inaccurate or incomplete.
  • Accountability: You have the right to know who the designated Privacy Officer is and to make an inquiry or complaint about our privacy practices.

5. Safeguards and Security

We protect your PHI with administrative, technical, and physical safeguards:

  • Administrative: All staff sign confidentiality agreements, receive regular privacy training, and are only permitted to access information on a "need-to-know" basis.
  • Technical: Electronic records are protected by passwords, encryption, secure networks, and regular security updates.
  • Physical: Paper records (if any) are secured in locked cabinets, and access to the clinic premises is restricted.

7. Complaints

If you believe your privacy rights have been violated, you may file a complaint directly with our Privacy Officer. If your concerns are not addressed to your satisfaction, you have the right to complain to the Information and Privacy Commissioner of Ontario (IPC):
